Key Responsibilities\n

• Conduct initial client meetings to identify their goals, preferences, and available support options, ensuring that each individual’s journey begins with a personalized approach.

• Design and implement comprehensive rehabilitation plans tailored to clients’ education levels, capabilities, and future aspirations, adapting these plans as necessary throughout the treatment process.

• Collaborate closely with an interdisciplinary team, including physicians, occupational therapists, and psychologists, to facilitate holistic rehabilitation programs that effectively meet clients’ mental health needs.

• Maintain up-to-date case files and records, ensuring accurate documentation of client histories, services rendered, insurance details, and ongoing correspondence with clients.
\n
• Regularly monitor and document client progress, making necessary adjustments to rehabilitation goals to ensure their successful outcomes.
\n
• Stay actively engaged with clients during job training and occupational placement, addressing any concerns or placement challenges in real time to ensure seamless transitions.
\n
• Work alongside clients’ families to optimize support systems and address client needs outside of scheduled sessions.
\n
• Timely assess client needs and prepare comprehensive reports that inform ongoing therapeutic strategies and interventions.
\n
• Make specialized recommendations and referrals to external resources to enhance clients’ care and recovery process when necessary.

Patient Population / Client Focus\n

In this role, you will primarily work with clients who are facing mental health and mood disorders within an outpatient setting. Your interventions will not only address individual therapeutic needs but also aim to develop client independence through structured support and effective rehabilitation strategies.

Qualifications\n

• A Master’s Degree in a relevant field such as counseling, psychology, social work, or marriage and family therapy.

• Valid licensure as a Licensed Professional Counselor (LPC) in the state of Connecticut.
\n
• Demonstrable experience in mental health treatment, specifically with clients experiencing mood and mental health disorders in either residential or outpatient environments.
\n
• Team-oriented mindset with a strong commitment to our mission of facilitating growth and positive change in the lives of clients.

• Excellent clinical skills, including proficiency in motivational interviewing, cognitive-behavioral therapy, dialectical behavioral therapy, and meticulous documentation practices.

We highly value candidates with prior exposure working in multidisciplinary teams, especially within the context of outpatient services. Familiarity with treatment methodologies specific to rehabilitation and behavior management is a strong plus.
