Key Responsibilities\n

• Conduct comprehensive assessments of each client's psychosocial needs, taking into account their emotional, mental, and social circumstances to develop individualized support plans.

• Work collaboratively with clients and their families to craft personalized care strategies that address specific recovery goals and preferences.

• Facilitate counseling sessions, providing emotional support and guidance as clients navigate their recovery journey, and help them manage the challenges that arise during detoxification.

• Advocate tirelessly for clients' rights, working to ensure that they have access to essential resources and support both within and outside our organization.

• Engage regularly with team members—including nurses, addiction specialists, and counselors—to coordinate care efforts effectively and adaptively to address clients' evolving needs.

• Perform regular follow-up calls and virtual check-ins to monitor clients' overall well-being and assess their living conditions, intervening when needed to ensure safety and support.

• Provide education about available community resources, recovery programs, and support networks that can enhance clients' social engagement and recovery outcomes.

• Maintain accurate and comprehensive documentation of assessments, care plans, and progress notes to reflect every client interaction and ensure effective communication within the team.

• Stay updated on the latest practices in social work and substance use treatment to ensure compliance with ethical standards and contribute to best practices in client care.

Qualifications\n

Compensation and Benefits\n

Compensation for this position ranges from $60,000 to $100,000 annually, depending on experience and qualifications.
