Key Responsibilities\n

As a Telehealth LPC, your key responsibilities will include:

• Conducting thorough patient intakes that focus on diagnostic and functional evaluations, while making informed recommendations for treatment goals and plans to primary care providers.

• Delivering brief yet impactful interventions for patients seeking mental health services, with a focus on fostering emotional growth and stability.

• Collaborating closely with primary care providers to identify patients requiring referral to specialty mental health services, ensuring seamless transitions in care.

• Monitoring patient progress through follow-up check-ins to evaluate treatment effectiveness and make necessary adjustments to plans.

• Developing individualized relapse prevention plans and teaching patients self-management skills to encourage independence in their recovery journeys.

• Maintaining detailed clinical documentation of patient progress while adhering to the highest standards of confidentiality and ethical conduct.

• Offering consultations to clinic management and other team members about behavioral health services and areas for improvement.

Qualifications\n

The successful candidate will possess:

Compensation and Benefits\n

Molina Healthcare & Wellness Center offers a competitive salary range of USD 87,000 – 105,000 per year, commensurate with experience.
