About the Program Health Homes is an outreach program which engages clients (including pediatrics) in the community with health and social services to best meet their needs. Health Home (HH) Services are intensive services that coordinate care across several domains. The purpose is to coordinate clinical and social service expertise for high risk clients with complex chronic conditions, mental health challenges, substance use disorder issues, and/or long term service needs and supports. Duties & Responsibilities Work as a member of a multidisciplinary Health Homes Team Assist Program Coordinator to process referrals received from Lead Entities (North West Regional Council, Molina, Amerigroup, CHPW, etc.) Assist Program Coordinator and Care Coordinators in screening and generating referrals where appropriate Assist Program Coordinator in ‘smart assignment’ of clients to Care Coordinators Assist in engaging hard to reach clients, establishing rapport, and making arrangements for Care Coordinators to have in person contacts Carry out pre-engagement activities with clients and schedule face-to-face appointments for Care Coordinators in the field Engage with clients via telephone and mail out activities to secure and confirm appointments with Care Coordinators Assist colleagues to obtain necessary consents and assist in identifying initial client objectives and goals identified at referral and during initial telephone outreach Assist Program Coordinator in preparation for site visits and audits Education & Experience Bachelors level degree in related mental health field with 2 years’ experience Behavioral Health Professional or Specialist – Mental Health Counsellor with Masters level degree in related field, Certified MH Counselor with Bachelors level degree in related field, or Agency Affiliated Counsellor with Bachelors level degree in psychology Registered Nurse / Nurse Practitioner / Licensed Practical Nurse Social Worker – Masters or Bachelors in social work, human services or related field Substance Use Disorder Professional Licensure & Certifications Washington state driver’s license with acceptable driving record and required $50,000 automobile liability insurance First Aid and CPR certification Licensure related to qualifications listed above Other Considerations DSHS background clearance including fingerprint $50,000 automobile liability insurance required Strong verbal and written communication skills Skilled with Microsoft Office Suite Ability to lift approximately 20-30 pounds and physically assist unsteady clients Compensation $20.24 – $22/hour, determined by applicable industry experience, education, and certifications/licensure Paid vacation – earned at .04 per hour worked for hourly employees, and 6.67 per month for salary (equivalent to about 2 weeks per year for full-time employees) Sick Leave – 1 hour per 40 hours worked (equivalent to about 52 hours for full-time employees) Holiday pay, with 8 holidays observed Paid “Floating” Holiday to be used on a day of choice Health Savings Account Health Reimbursement Arrangement Basic Life & AD&D insurance WA Paid Family Medical Leave Employee Assistance Program Pet Insurance Legal & IDShield Accident, Critical Illness, & Hospital Insurance 401K & matching up to 6% Life Insurance College savings plan To comply with Federal law, Sunrise Services participates in E-Verify.
