Client Care Coordinator — Home Care Agency

Acti-Kare is a locally owned, women-led home care agency that has been serving families across the greater Seattle area since 2016. We provide in‑home care for seniors and adults who need support — personal care, companion care, dementia care, and more — across King, Snohomish, and Pierce Counties. About the Role This is a client services and coordination role at the center of how we deliver care. You will be the client advocate: the person families hear from when they are trying to figure out how to get their loved one the help they need, and the person who makes sure the care that follows actually reflects what was promised. That requires warmth, professionalism, organizational discipline, and the ability to hold a difficult conversation without losing your steadiness. The field support component of this role — conducting caregiver introductions and trainings at client homes and providing occasional emergency shift coverage when needed — is a real and meaningful part of the job. It is also what makes the best coordinators in this field so effective: when you understand care from the inside, you are better at every other part of this job. We prefer candidates who hold an active Washington State HCA or CNA credential and bring hands‑on caregiving experience, though we will consider exceptional candidates from healthcare coordination or related fields. What you will be doing Conducting in‑home client intakes and assessments, developing individualized care plans in collaboration with clients and families Working with the admin team to match caregivers to clients based on fit, conducting in‑home introductions, and walking through care plans before service begins Managing ongoing client relationships — checking in, responding to concerns, coordinating care adjustments, and monitoring quality and consistency across your caseload Building relationships with new client leads, including scheduling and conducting initial in‑home visits Maintaining accurate, compliant client records in our care management software Conducting hands‑on coaching and introductions with caregivers at client homes, particularly for new placements Providing emergency field shift coverage when a scheduled caregiver cannot report and no backups are available Ensuring adherence to Washington State home care regulations and internal standards Who we are looking for Our top priority is a team member with a genuine service orientation and the character to match. On experience and credentials: 1 or more years in a client‑facing professional role — care coordination, case management, medical office, community health, social services, or equivalent Natural ability to build trust with people during stressful or emotional situations Professional presence and communication style appropriate to an in‑home setting Strong organizational skills and the ability to manage multiple priorities without losing the details Valid Washington State driver’s license, reliable transportation, and willingness to travel throughout King, Snohomish, and Pierce Counties Proficiency in Microsoft Office; willingness to learn our care management software Ability to pass a criminal background check and provide proof of auto insurance Preferred Active Washington State HCA certification or CNA license in good standing 1‑2 or more years of direct caregiving experience Experience in non‑medical home care agency operations, or similar field Background in caregiver training, mentoring, or field supervision Experience in a client services, patient services, or intake coordinator role What We Offer $29–$34 per hour, based on experience Team bonus options Medical insurance, dental insurance, and vision insurance Paid time off and paid sick leave Continuing education credits (HCA and CNA) A genuinely supportive team and a workplace where your work matters We are a team of people who love what we do and take it seriously.
