Lead Care Manager

Anchor House | Sacramento, CA 95621 Anchor House is seeking a Lead Care Manager to support high-need Medi-Cal members through the CalAIM Enhanced Care Management (ECM) program. This role focuses on overseeing care coordination, supporting care managers, and ensuring quality service delivery. Responsibilities Lead and support care managers to ensure effective care coordination Oversee assessments, care planning, and service coordination under CalAIM ECM Collaborate with the ECM Program Manager and external providers Monitor quality measures and reporting requirements Identify program improvements and communicate updates to the team Qualifications 3+ years of relevant experience required High school diploma or GED required Knowledge of Medi-Cal, CalAIM, or community services preferred Strong communication and organizational skills Proficiency with Google Workspace and basic office tools Flexible availability, including occasional evenings or weekends Multilingual skills a plus Compensation & Schedule $18-$22 per hour Full-time, Monday-Friday (on-call/weekend opportunities) Paid time off Location On-site, Sacramento, CA 95621 #J-18808-Ljbffr
