- # Physical Therapist (Outpatient) Wexford Health + Wellness PavilionApplyremote type: On-sitelocations: Wexford PA, 15090, 12311 Perry Highwaytime type: Full timeposted on: Posted Todayjob requisition id: J283844## **Company :**Allegheny Health Network## **Job Description :****Rotational hours - daylight with some evenings****Scheduled 4 days between M-F with flex day off, no weekends****(2) 12 hour days and (2) 8 hour days****GENERAL OVERVIEW:**Evaluates and treats patients with injuries or diseases resulting in loss of physical or functional abilities; supervises work performed by support personnel; effectively communicates and collaborates with patients, families and members of the healthcare team.**ESSENTIAL RESPONSIBILITIES:*** Performs initial and on-going assessments of patient's condition and establishes goals and plan of care which is appropriate to problems identified and involves the patient/family according to their capabilities and desires.* Performs therapy interventions utilizing standard, and, possibly specialized, physical therapy techniques and skills as appropriate for the age and condition of the patient.* Completes documentation to comply with department, hospital and regulatory standards.* Participates in departmental initiatives for operational improvement, including committees and educational offerings.* Performs other duties as assigned or required.**QUALIFICATIONS:**Minimum* Bachelor’s Degree* State licensure as a Physical Therapist or eligible.* CPR – American Heart Association* Act 34 Criminal Background Clearance Certificate* Act 33 Child Abuse Clearance Certificate* Act 73 FBI Fingerprinting Criminal Background Clearance CertificatePreferred* Master’s Degree.* Current state driver’s license.***Disclaimer:*** *The job description has been designed to indicate the general nature and essential duties and responsibilities of work performed by employees within this job title.
