Work Environment

This position offers a remote work setting, allowing for flexibility in your schedule while maintaining consistent contact with clients and their families. You'll collaborate with a team of healthcare professionals, including nurses, caregivers, and other social workers, all dedicated to supporting the unique needs of seniors. Although the role primarily involves remote work, you may occasionally make in-person visits to clients in their homes or community settings to provide comprehensive care.

Key Responsibilities

As a Licensed Master Social Worker, your daily activities will include:

• Conducting detailed psychosocial assessments to evaluate seniors' emotional, mental, and social needs.
• Developing personalized care plans in collaboration with clients and their families to align with their goals and preferences.
• Providing counseling and emotional support to assist seniors in managing life transitions such as loss, health changes, and moving into assisted living.
• Advocating for clients’ rights and ensuring they have access to necessary community resources, services, and support systems.
• Participating in interdisciplinary team meetings to review client progress and modify care plans as needed.
• Making regular home visits to monitor clients' well-being, assess living conditions, and ensure safety.
• Facilitating communication among family members involved in care, addressing conflicts with sensitivity.
• Educating clients and their families about community resources and support options to encourage engagement and social interaction.
• Maintaining comprehensive documentation of assessments, care plans, and client interactions to ensure continuity of care and compliance with ethical standards.

Patient Population / Client Focus

Your primary focus will be the older adult population in our community. You will engage with seniors facing various challenges, from mental health issues and chronic illnesses to socio-economic barriers. Understanding their unique experiences and providing tailored support is essential to fostering a positive impact on their quality of life.

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• Current and unrestricted licensure as a Licensed Master Social Worker (LMSW) in the state of Colorado.
• A minimum of two years of progressive experience in a social work capacity, particularly in settings that serve older adults.
• In-depth knowledge of aging-related issues, resources, and best practices in social work.
• Exceptional assessment, counseling, and rapport-building skills.
• Strong communication and collaboration capabilities to work effectively within multidisciplinary teams.
• Familiarity with electronic health record (EHR) systems and general computer applications.

Preferred Experience

While not mandatory, candidates with experience in a community health clinic or geriatric settings will be viewed favorably. Additional certifications in gerontology or relevant therapeutic modalities can enhance your application.
