Work Environment\n

This position is fully remote, allowing flexibility for the qualified candidate to manage their schedule effectively. You will primarily engage with clients via telehealth platforms, ensuring continuity of care and accessibility for seniors who may have mobility issues. Working hours are typically Monday to Friday from 8:00 AM to 4:30 PM, with occasional evening or weekend hours required based on client needs. Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, identifying emotional, mental, and social factors that influence well-being.

• Develop and implement individualized care plans with seniors and their families, addressing their unique goals and preferences.

• Provide one-on-one counseling and emotional support to help clients navigate challenges such as grief, loss, and health transitions.

• Act as an advocate for clients, facilitating their access to community resources, and ensuring they receive the necessary support services.

• Collaborate with interdisciplinary teams—including healthcare professionals, caregivers, and other specialists—to provide holistic care to clients.

• Conduct regular follow-up visits with clients to monitor progress, address safety concerns, and make necessary adjustments to care plans.

• Facilitate communication among families involved in the care of clients, addressing any conflicts and promoting collaboration.

• Educate clients and families about community resources, available support networks, and programs designed to enhance social engagement.

• Maintain accurate and timely documentation of assessments, care plans, and client interactions, complying with all ethical standards.

Qualifications\n

• A Master's degree in Social Work from an accredited program.

• A current, valid state license as a Clinical Social Worker (LCSW-C).

• A minimum of two years’ experience in a social work role, with a preference for experience in community or geriatric settings.

Compensation and Benefits\n
