Key Responsibilities\n

• Conduct comprehensive psychosocial assessments using interviews, clinical observations, and thorough reviews of patients’ medical records to evaluate emotional, social, and environmental factors critical to their recovery.

• Facilitate counseling sessions for patients and their families, providing expert support for adjustment issues, mental health challenges, and emotional well-being throughout the rehabilitation process.

• Collaborate closely with an interdisciplinary rehabilitation team to develop and implement individualized treatment plans that address specific psychosocial needs and milestones for each client.

• Assist patients in navigating and accessing community resources essential for continued support, such as vocational rehabilitation programs, support groups, and housing assistance.

• Educate patients and families about available community resources and post-rehabilitation services, ensuring they have the tools necessary for a successful transition.

• Participate actively in discharge planning, assisting in the seamless transition of patients to home or other care settings, and facilitating ongoing support as needed.

You will be working primarily with a diverse client base, including individuals facing substance use disorders and other mental health challenges. Qualifications\n

Compensation and Benefits\n
