Senior Care Solutions is seeking a compassionate, detail-oriented Baylor Intake Coordinator to join our growing home health team. In this role, you\'ll help ensure patients receive timely, high-quality care by coordinating referrals, processing admissions, and collaborating with our clinical teams throughout the weekend. Responsibilities Process and coordinate new home health referrals and admissions. Verify patient information and ensure all required documentation is complete. Communicate with physicians, hospitals, case managers, and referral sources. Coordinate with clinical staff to facilitate timely Start of Care visits. Maintain accurate patient records in the EMR system. Monitor referral status and ensure compliance with Medicare and agency guidelines. Provide exceptional customer service to patients, families, and referral partners. Collaborate with the weekday intake team to ensure seamless continuity of care. Qualifications Previous intake, scheduling, or healthcare administrative experience. 1+ years of Home health/Medicare experience. Strong organizational and multitasking skills. Excellent verbal and written communication. Proficiency with Microsoft Office; EMR experience preferred (WellSky/Kinnser experience is a plus).
