Physical Therapist (PT) Home Health

Schedule: Shift: 8-hour days (08:00 - 16:30). This facility cannot accommodate 4x10s or 3x12s.

Assignment Details: Guaranteed hours: Facility can call off up to one shift per pay period. Expected to visit up to 6 patients per day.

Requirements: 1 year of skilled home health experience required. BCLS certification required. Oasis documentation experience required. Pending license accepted. Traveler is expected to have their own transportation.

Responsibilities: Provide patient evaluations and develop treatment plans. Administer therapeutic exercises and modalities as prescribed. Document patient progress and adapt interventions as necessary. Educate patients and families on home exercise programs. Collaborate with other healthcare professionals to ensure optimal patient care.
