Registered Nurse (RN) - Pediatric & Adult Home Health | Private Duty Nursing

Responsibilities Provide personalized nursing care for pediatric and/or adult patients with complex needs in the home setting. Coordinate care plans with physicians, therapists, and other team members. Monitor patient progress and update care plans as needed. Educate and support families with in-home care techniques. Document visits and care notes in EMR system. Collaborate with clinical supervisor and participate in case conferences. Act as an advocate for patient welfare and coordinate care between patients, families, agency, and other health providers. Perform appropriate preventive and rehabilitative nursing procedures. Prepare clinical and progress notes for each patient visit and summarize them. Attend case conferences as assigned. Inform physician and personnel of changes in patient condition and needs. Educate families on home health strategies and applicable community resources. Observe and report patient symptoms, reactions to treatments, medications, and changes in physical or emotional condition. Perform medication reconciliation and administer medication per agency policy and physician orders. Provide written instructions for aides or other ancillary disciplines caring for the patient. Process orders and notify physician of patient needs and changes in condition. Refer cases to PT, Speech/Language, OT, and MSW as required. Maintain confidentiality and adhere to agency policies. Takes on-call duty nights, weekends, and holidays as assigned; act as after-hours supervising nurse. Report to Nursing Supervisor any incidents, variances, or complaints. Maintain attendance status, professional licensure, CPR, and continuing education. Assist in clinical record and utilization review for QAPI. Perform additional duties as assigned. Qualifications Active RN license in the state of practice. CPR certification. Clinical experience preferred; new grads welcome. Excellent communication and critical‑thinking skills. Confidence, compassion, and calm presence during routine and critical moments. Ability to organize work procedures, assume responsibility, and supervise ancillary disciplines. Physical ability to perform described duties and travel as required. Proficient with digital communication and EMR. Excellent verbal and written communication. Dedication to patient‑centered care in home environment. Must be able to maintain licensure and meet all health requirements.
