Key Responsibilities\n

• Engage with a multi-disciplinary team composed of healthcare providers, nurses, and chaplains to create and implement comprehensive care plans tailored to the unique needs of each patient.

• Conduct assessments to evaluate clients’ psychosocial needs and develop individualized treatment approaches that address mental health challenges.

• Provide counseling and support to patients and their families, aiding them in navigating emotional responses and decision-making related to end-of-life care.

• Educate clients on available mental health resources and services, empowering them to make informed choices about their care.

• Maintain up-to-date knowledge of best practices in mental health treatment, legal requirements, and hospice care policies to ensure adherence to standards and regulations.

The primary focus of your work will be elderly patients in hospice care and their families. Qualifications\n

• Master’s degree in Social Work from an accredited institution.
