Zenith Mind Counseling Center is seeking a dedicated Licensed Clinical Social Worker (LCSW) to join our team in a full-time capacity at our Hospice Center in Macon, Georgia. This remote position plays a crucial role in providing compassionate and high-quality mental health support to clients, particularly older adults, ensuring their emotional and psychological needs are met during critical life transitions.

Work Environment\nAs part of our remote work model, you will have the flexibility to conduct sessions and support families from the comfort of your own home while being closely integrated with our team of professionals. You will work Monday through Friday from 9:00 AM to 5:00 PM, enabling a work-life balance that can accommodate personal needs and commitments. Key Responsibilities\nIn your role as a Licensed Clinical Social Worker, your responsibilities will include but are not limited to:

- Acting as a pivotal liaison between clients, families, and outpatient providers to ensure seamless communication and care coordination. \n
- Engaging with insurance companies to facilitate pre-certification processes and manage utilization effectively, ensuring clients receive necessary services without unnecessary delays. \n
- Participating in comprehensive discharge and aftercare planning, ensuring clients transition smoothly from hospice care to their next phase of support. \n
- Providing therapeutic meal support, where applicable, while modeling and fostering healthy relationships with food among clients. \n
- Delivering empathetic and affirming care tailored to each client's unique identity and circumstances, respecting their diverse backgrounds and experiences. \n
Your primary focus will be on serving older adults and their families navigating end-of-life issues, mental health challenges, and emotional distress. Your expertise will help clients cope with fear and uncertainty, providing them with the support needed to live their remaining years with dignity and grace.
Qualifications\nTo be successful in this role, candidates must have:

- A Master's degree in Social Work from an accredited institution. \n
- A valid Licensed Clinical Social Worker (LCSW) license in the state of Georgia. \n
- Strong clinical and diagnostic skills to assess and address a wide range of mental health issues. \n
- Excellent interpersonal and communication skills, allowing for effective collaboration with clients and fellow professionals. \n
- The ability to work independently while also contributing positively to a team environment. \n
Preferred qualifications include:
