The work setting combines both office and community interactions, ensuring a comprehensive support approach for our clients. Key Responsibilities Directly engage with clients in both office and community settings, assessing and addressing their psychological needs. Foster collaborative relationships with team members to promote seamless service delivery and achieve common objectives. Conduct comprehensive, evidence-based mental health screenings and facilitate detailed assessments as necessary. Design and implement integrated care plans, ensuring clients have access to essential linked services. Deliver focused prevention, treatment, and recovery services, adjusting strategies based on each client’s unique presenting issues. Assist in crisis intervention efforts, providing urgent support as required. Monitor client caseloads, ensuring timely interventions and adherence to treatment goals. Complete necessary documentation, including reports and data submission, to maintain compliance and quality standards. Participate in on-call coverage, ensuring that client needs are addressed during and after program hours as assigned. Collaborate on internal committees aimed at enhancing agency service delivery and quality improvement efforts. Patient Population / Client Focus The role primarily focuses on clients experiencing symptoms of major mental illness and severe emotional disorders. Your expertise in assessment, diagnosis, and treatment will be crucial in providing targeted support that respects the diverse backgrounds and experiences of our clients. Qualifications A valid Licensed Clinical Social Worker (LCSW) credential. A Bachelor's degree in Social Work or a related field is required. At least 2 years of relevant experience in a clinical setting working with mental health populations. Exceptional verbal and written communication skills. Strong organizational and planning abilities. Comprehensive knowledge of assessment, diagnosis, treatment methodologies, and crisis management protocols. Capable of working effectively within the organizational and financial frameworks of the community health center. Preferred Experience Experience in integrated care settings or community health clinics. Familiarity with various therapeutic approaches and practices tailored to different age groups and cultural backgrounds. Proficient in using electronic health records (EHR) systems for maintaining client data. Compensation and Benefits The salary range for this role is competitive, between USD 70,000 and 85,000 annually, commensurate with experience.
