Community Health Navigator for Homeless Services
St. Jude Neighborhood Health Center
St. Jude Neighborhood Health Center in California is seeking a Community Health Worker to provide Enhanced Care Management to individuals experiencing homelessness and other needs. The role involves assessing needs over the phone and connecting patients to resources while promoting health education.
