Registered Nurse - LTC/SNF

Job Summary:

Provides comprehensive nursing care to residents in a long-term care or skilled nursing facility setting. Responsible for assessing, planning, implementing, and evaluating resident care in collaboration with the healthcare team.

Responsibilities:

• 
  Administer direct and indirect nursing care in accordance with the physicians plan of care.
  
  
• 
  Conduct resident assessments, diagnoses, and documentation to ensure accurate and timely care delivery.
  
  
• 
  Collaborate with multi-disciplinary teams to optimize resident outcomes and ensure coordinated care.
  
  
• 
  Support quality improvement initiatives to enhance resident safety and care standards.
  
  
• 
  Maintain up-to-date knowledge of residents' age and cultural needs to personalize care.
  
  
• 
  Ensure compliance with all relevant regulations, policies, and licensing requirements.
  
  

Qualifications:

• 
  Hold an active California RN license.
  
  
• 
  Possess a minimum of one year of experience as a registered nurse.
  
  
• 
  Graduate from an accredited nursing program.
  
  
• 
  Maintain current CPR certification.
  
  
• 
  Demonstrate strong assessment, communication, and critical thinking skills.
