Job Description

As an LVN/LPN in a Skilled Nursing Facility (SNF), you will deliver high-quality patient care within a high-demand environment. Working under direct supervision of an RN while autonomously practicing within the LVN/LPN scope, you will manage a typical patient load of five patients, ensuring safe and effective clinical support. Your role will encompass patient assessment, medication administration, wound care, and collaboration with healthcare staff to promote optimal patient outcomes.

 

Responsibilities

• Provide comprehensive patient care in a skilled nursing setting
• Administer medications accurately and on schedule
• Monitor and record vital signs regularly
• Perform wound care and manage related documentation
• Develop and update care plans based on patient assessments
• Conduct thorough patient assessments to track health status
• Communicate effectively with nursing staff and other healthcare professionals
• Implement infection control protocols to ensure patient safety
• Educate patients on health management and clinical procedures

 

Required Qualifications

• Minimum 1 year of experience in licensed vocational/practical nursing
• Completion of an accredited LVN/LPN program
• Current and active LVN or LPN license

 

Preferred Qualifications

• Basic Life Support (BLS) certification
• Strong patient safety awareness
• Proficiency in clinical documentation
• Advanced skills in medication administration and wound care
• Knowledge of infection control best practices
• Excellent communication and teamwork abilities
• Critical thinking and problem-solving skills
