Heritage Wellness & Rehabilitation is seeking an experienced Admissions Coordinator to join our team!

As an Admissions Coordinator, you will help manage the admissions process while developing strong relationships with prospective residents, families, hospitals, and referral partners.

• Coordinate the admissions process in accordance with facility guidelines, policies, and procedures.

• Respond to phone calls, walk-in inquiries, and admission requests in a timely and professional manner.

• Provide prospective residents, families, and referral sources with information about our services, programs, accommodations, and rates.

• Conduct tours and answer questions about the facility and available services.

• Collect and accurately document required admission information, including insurance coverage, physician information, resident needs, and admission dates.

• Coordinate financial verification and required documentation before admission.

• Collaborate with nursing, Social Services, hospital staff, and other departments to support appropriate admissions.

• Help ensure rooms and necessary arrangements are prepared before a resident’s arrival.

• Build and maintain positive relationships with hospitals, healthcare professionals, referral sources, residents, and families.

• Promote facility programs and services to prospective residents and referral partners.

• Identify opportunities to develop census and maintain a strong quality mix.

• Maintain knowledge of referral trends, resident origins, and local healthcare resources.

• Follow up on inquiries and referral opportunities to support continued census growth.

• Educate residents and families about available transition and care options.

• Support residents and families throughout the admission and transition process.

• Address questions, concerns, and dissatisfaction in a professional and timely manner.

• Investigate concerns, coordinate follow-up, and recommend solutions when needed.

• Assist with post-admission and discharge satisfaction calls.

• Help create a welcoming experience for residents and their responsible parties.

• Participate in admissions meetings and collaborate with facility leadership.

• Ensure admissions documentation is accurate, complete, and maintained appropriately.

• Follow facility policies, admission criteria, and applicable healthcare requirements.

• Work closely with the interdisciplinary team to support quality resident care.

• Perform other duties as assigned.

• High school diploma or equivalent required.

• Previous experience in healthcare, skilled nursing, long-term care, hospital admissions, patient intake, or healthcare marketing preferred.

• Strong communication, organization, and customer service skills.

• Ability to build positive relationships with residents, families, and referral sources.

• Professional appearance and a compassionate, service-focused approach.

• Ability to manage multiple priorities while maintaining accurate documentation.

• Knowledge of long-term care or healthcare services is a plus.
