Nurse Practitioner (NP) – Nursing Home Primary Care (Weekend Coverage) Location: Brooklyn, NY (Canarsie) Schedule: Saturday & Sunday Compensation: $500 – $600 per day (1099) A medical group providing primary care services to nursing homes is seeking a Nurse Practitioner to provide weekend coverage for a skilled nursing facility located in the Canarsie section of Brooklyn. This role focuses on providing primary care to geriatric patients in a nursing home environment. The provider will manage patient visits, follow-ups, and coordinate care with facility staff to ensure continuity of care over the weekend. Schedule The provider will work weekends (Saturday and Sunday). Providers must be available between 8:00 AM and 6:00 PM , with flexibility in structuring their day based on patient volume and facility needs. Compensation $500 – $600 per day 1099 independent contractor Additional earning potential depending on patient volume Clinical Responsibilities Provide primary care services to geriatric nursing home residents Conduct routine and urgent patient visits Manage chronic conditions and acute medical issues Coordinate care with nursing staff and facility leadership Participate in CCM and RPM programs when applicable Document patient encounters in facility EMR systems (SigmaCare or PCC) Requirements Active New York Nurse Practitioner license Active Medicare and Medicaid provider IDExperience working with geriatric patients preferred Skilled Nursing Facility experience preferred RN background in a nursing home or long-term care setting is highly valued #J-18808-Ljbffr
