This Per Diem Physical Therapist role offers flexibility, autonomy, and the opportunity to provide high-quality rehab and community-based care directly to patients in their homes. As a Home Health Physical Therapist , you will work with senior care populations , including geriatric patients recovering from illness, injury, or surgery. Your focus will include fall prevention, strength building, mobility training, and functional independence for senior and geriatric patients . Key Responsibilities: Conduct in-home physical therapy evaluations and reassessments Develop and implement personalized rehabilitation care plans Improve strength, balance, mobility, and functional outcomes Provide therapy in home health, assisted living, and community care settings Educate patients and families on fall prevention and safe mobility techniques Document visits using WellSky (Kinnser) EMR Collaborate with interdisciplinary home health teams Qualifications Licensed Physical Therapist (PT) – California (Required) CPR Certification (AHA approved) Minimum 1 year of clinical experience (Home Health preferred) Experience in senior care, geriatrics, rehab, or community-based care preferred Strong clinical, communication, and patient education skills Valid driver’s license, reliable transportation, and auto insurance What You’ll Gain Hands-on experience in home health and rehabilitation care Work with geriatric and senior care populations Independence in a mobile, community-focused role Support from a mission-driven home health organization Pay-Per-Visit $120 - $150 USD Voluntary Self-Identification For government reporting purposes, we ask candidates to respond to the below self-identification survey.Completion of the form is entirely voluntary.
