Key Responsibilities\n

• Conduct thorough and comprehensive assessments of clients to identify their social, emotional, and environmental challenges—involving parents and guardians in the assessment process when appropriate.

• Develop individualized service plans that encompass measurable goals, interventions, and ongoing progress monitoring tailored to each client’s unique needs.

• Engage in providing counseling and therapeutic support to individuals and families facing a range of challenges including anxiety, depression, trauma, and family conflict.

• Collaboratively work with clients in crafting personalized service plans, ensuring connections to relevant community resources such as educational support, housing assistance, and vocational training.

• Implement crisis intervention strategies, conduct risk assessments, and develop safety plans in coordination with clients during critical moments.

• Establish strong partnerships with community agencies and organizations to coordinate services effectively and advocate for the best interests of clients.

• Participate actively in community outreach initiatives aimed at raising awareness about mental health issues and promoting the programs and services offered by Life Line Counseling.

• Engage in regular case conferences, team meetings, and opportunities for professional development through staff training.

Patient Population / Client Focus\n

As a Mental Health Therapist at Life Line Counseling, you will be primarily focused on school-aged children and their families within the San Jose area, helping them navigate emotional and behavioral challenges that can impact their educational success and overall well-being. Your role will be critical in addressing issues ranging from peer relationships to family dynamics, thereby fostering resilience and growth in young clients.

Qualifications\n

• Master's degree in Social Work from an accredited program.
