Work Environment\n

The work setting for this position is entirely remote, allowing for flexibility while ensuring that our clients receive the care they deserve. Your schedule will primarily be Monday through Friday, 8:00 AM to 4:30 PM, with occasional evening or weekend hours as necessary to meet client needs.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial needs, addressing emotional, mental, and social considerations specific to their circumstances.

• Develop and implement individualized care plans in collaboration with clients and their families, focusing on their goals and preferences.

• Provide ongoing counseling and emotional support to seniors and their loved ones during life transitions, such as relocation to assisted living or dealing with chronic health issues.

• Advocate for clients’ rights and access to essential community resources, ensuring they benefit from relevant services and programs.

• Work closely with an interdisciplinary team to coordinate care efforts, including nurses, caregivers, and other social service professionals.

• Conduct home visits, assess living conditions, and provide solutions aimed at enhancing safety and comfort.

• Facilitate open communication and conflict resolution among family members involved in the client’s caregiving process.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

Compensation and Benefits\n

The annual salary for this LCSW position ranges from USD 60,000 to 100,000, commensurate with experience and qualifications.
