Key Responsibilities\n

As a Licensed Clinical Social Worker within CHDC, your primary responsibilities will include:

• Assisting mental health consumers in navigating the complexities of both physical and behavioral health care systems to promote integrated care.

• Educating patients about available community resources and services, fostering their engagement in preventative measures, wellness promotion, and healthy lifestyle choices.

• Actively participating in multidisciplinary treatment team meetings to discuss patient progress and strategies for holistic care.

• Reporting any crisis events or unusual incidents promptly to the Medical Director, with immediate escalation of any psychiatric or medical emergencies to the Chief Medical Officer.

• Maintaining detailed and accurate documentation in clinical records, ensuring compliance with established procedures.
\n
• Engaging in quality improvement programs and attending necessary training sessions to stay updated with best practices.
\n
• Adhering to all CHDC policies and protocols while functioning as part of our integrated Primary Care Behavioral Health model.

• Utilizing evidence-based screening tools effectively to assess and address the healthcare needs of patients.
\n
• Collaborating with the Center of Excellence Team to provide targeted support to patients dealing with Opioid Use Disorder.
