Key Responsibilities\n

• Conduct comprehensive clinical evaluations to assess patient needs and formulate individualized therapy plans by actively engaging with patients during initial sessions.

• Lead structured psychoeducational and process-oriented therapy groups, facilitating discussions that encourage participation and provide valuable coping strategies.

• Initiate and coordinate discharge planning on the day of admission and conduct follow-up sessions to ensure patients understand their coping strategies, transition plans, and community resources available to them.

• Ensure effective communication with the Utilization Review Department and Business Department regarding necessary authorizations and payment processes.

• Participate collaboratively with outpatient providers and referral sources throughout the patient’s treatment journey to maintain continuity of care.
\n
• Demonstrate effective crisis management techniques during sessions and within the therapy milieu, advocating for patient needs while supporting milieu staff in their interactions.

• Assist in facilitating patient transfers to higher levels of care as needed, ensuring patients receive the necessary support as their treatment plans evolve.
\n
• Document services rendered, including initial assessments, treatment plans, and clinical case notes, in compliance with required timelines to maintain accurate patient records.
\n
• Contribute to clinical outreach initiatives and represent our clinic in community program tours, emphasizing our commitment to mental health awareness and support.
