Key Responsibilities\n

As a Licensed Clinical Social Worker, your role will be instrumental in delivering clinical behavioral health services aligned with the standards and goals of our health center. Your primary responsibilities include:

• Guiding mental health consumers through the complexities of both physical and behavioral healthcare systems, ensuring a seamless integration of services that promote whole-person health.

• Providing educational resources to patients on the various services available, encouraging their active involvement in disease prevention practices and the promotion of a healthy lifestyle.

• Attending multi-disciplinary treatment team meetings to collaborate on patient cases and develop holistic care plans.
\n
• Reporting any crises or unusual incidents promptly to the Medical Director and addressing psychiatric emergencies immediately with the Chief Medical Officer.

• Ensuring meticulous documentation of patient interactions and clinical records in accordance with established protocols.
\n
• Actively participating in quality improvement initiatives and contributing to continued staff training sessions aimed at enhancing service delivery.
\n
• Upholding compliance with all organizational policies and procedures while utilizing the integrated Primary Care Behavioral Health model.
\n
• Administering evidence-based assessment tools to evaluate patient healthcare needs and interpretation of results for care planning.
\n
• Collaborating closely with our Center of Excellence Team in addressing and managing patients with Opioid Use Disorder, ensuring they receive comprehensive care.

Patient Population / Client Focus\n

In this role, you will work with a diverse patient population that includes individuals from various backgrounds facing a range of mental health challenges, including those with co-occurring substance use disorders. Our focus is to provide empathetic, person-centered care that respects each patient's unique circumstances and fosters their resilience and recovery.

Qualifications\n

To be considered for this position, candidates must meet the following qualifications:

• A master’s degree in social work and possession of a valid Licensed Clinical Social Worker (LCSW) designation.

• A minimum of 2 to 3 years of relevant experience providing direct clinical services, particularly in behavioral health.

• A background in a medical setting, such as primary or specialty care, is preferred, along with the ability to collaborate effectively with interdisciplinary teams.

• This role requires occasional travel between CHDC sites for meetings, training sessions, and outreach activities.
