Work Environment

This position is primarily onsite, as you'll be conducting therapy sessions in the patient's home, which allows for direct interaction in a comfortable and familiar environment for our clients. Our team operates Monday through Friday, 9 AM – 5 PM, ensuring a balanced work-life schedule. Key Responsibilities

• Administer direct patient care, following the written and signed prescription from referring medical practitioners, ensuring adherence to all regulatory and best practice guidelines.
• Conduct comprehensive evaluations using established techniques to assess patient conditions effectively, tailoring assessments based on individual needs and progress.
• Design and implement customized treatment plans aimed at achieving specific rehabilitation goals, routinely modifying them based on patient observations and progress updates.
• Document and maintain accurate records of patient progress and treatment outcomes in accordance with departmental policies, ensuring confidentiality and compliance.
• Engage in critical real-time assessments during therapy sessions to modify treatment methodologies as necessary, addressing any complications that may arise during patient care.
• Participate in quality improvement initiatives, including chart reviews and departmental activities to enhance patient care delivery and operational efficiency.
• Facilitate family meetings and interdisciplinary discussions, contributing insights that enhance patient management and advocate for optimal treatment outcomes.

Patient Population / Client Focus

The ideal candidate will work with a diverse patient population ranging from post-operative rehabilitation to chronic pain management, providing essential support for those recovering from injuries or surgeries. Our patient-centered approach ensures that each treatment plan respects individual values, preferences, and needs, allowing for tailored rehabilitation journeys.

Qualifications

• A valid New Jersey physical therapy license in good standing.
• A Bachelor's Degree in Physical Therapy or a related field is required, with a focus on evidence-based practice.
• A minimum of 2 years of clinical experience in a relevant setting is mandatory.
• Strong interpersonal skills and a compassionate demeanor are essential for fostering effective communication with patients and their families.

Preferred Experience

• Experience in home health care is a plus, providing familiarity with challenges and strategies specific to in-home patient support.

Compensation and Benefits

This position offers a competitive salary ranging from $90,000 to $100,000 per year, commensurate with experience.
