Flex Well Therapy Center in Hillsborough, New Jersey, is seeking a dedicated and compassionate Licensed Physical Therapist to join our team in an onsite home care setting. Work Environment

This role is primarily based onsite within the homes of our patients, aligning with our home care model that facilitates personalized rehabilitation services. The shift schedule spans Monday through Thursday from 3 PM to 7 PM, with a condensed four-hour shift on Fridays. This work setting allows for direct interaction with patients in the comfort of their homes, making a significant impact on their recovery and rehabilitation journey.

Key Responsibilities

• Conduct comprehensive evaluations to screen, test, and diagnose patients suffering from injuries, diseases, and disabilities, applying evidence-based physical therapy modalities.
• Develop and implement individualized treatment plans that reflect the patient's specific needs, goals, and physician recommendations.
• Educate and train patients and their caregivers on therapeutic exercises and treatment procedures to support recovery at home.
• Document all patient interactions and treatment plans comprehensively and timely to comply with regulatory standards.
• Maintain open lines of communication with patients, families, and healthcare professionals to foster a collaborative treatment environment.
• Supervise and coordinate the activities of physical therapy assistants and support staff, ensuring high standards of patient care.
• Participate in quality improvement initiatives to enhance treatment outcomes and patient satisfaction.
• Engage in continuing education and training opportunities to stay abreast of advancements in physical therapy practices.
• Contribute to program development efforts aimed at expanding our therapy services and enhancing community involvement.

Patient Population / Client Focus

Our therapy services cater to a diverse client base, including neonates, children, adolescents, adults, and geriatric patients. The ideal candidate will possess a strong understanding of how to tailor therapy interventions across these populations, taking into account varied therapeutic needs and expected outcomes. Your expertise will directly contribute to improving functional mobility and quality of life for individuals recovering from surgery, injury, or chronic conditions.

Qualifications

• Current state Physical Therapist license, demonstrating your ability to practice within New Jersey.
• A Bachelor's degree in Physical Therapy from an accredited institution is required, with a preference for candidates holding a Master's or Doctorate in Physical Therapy (DPT).
• A minimum of one year of clinical experience in a healthcare setting, preferably in home health care.
• Strong communication skills and the ability to engage effectively with diverse patient populations.
• Proficient in physical therapy practices, patient evaluation techniques, and therapeutic modalities.
• Physical ability to perform essential tasks, including lifting, standing, walking, and manual dexterity.

Preferred Experience

• Experience working with a variety of patient demographics, particularly within a home care environment.
• Knowledge of insurance documentation procedures and patient billing processes.
• Proficient in using electronic medical record (EMR) systems to document patient care appropriately.
