Key Responsibilities

• Conduct comprehensive evaluations of patients, including reviewing pertinent medical history and implementing specific assessments to determine functional limitations and create individualized treatment plans.

• Demonstrate proficiency in implementing treatment protocols tailored to the unique needs of each patient, continuously adapting approaches based on evaluation findings and patient progression.

• Document patient care activities meticulously, including detailed daily encounter notes, interim progress summaries, and discharge evaluations, ensuring compliance with evolving healthcare standards and regulations.

• Collaborate with other healthcare professionals to ensure integrated care while recognizing any changes in the patient's condition that might necessitate adjustments in their plan of care.

• Participate in scheduled facility meetings and contribute to team initiatives aimed at enhancing access to care and improving service delivery within the community.

• Engage in discharge planning to ensure patients receive appropriate follow-up care, facilitating a seamless transition from therapy to everyday activities.

• Maintain knowledge of and compliance with all corporate training requirements and regulations at the state and federal levels.

Patient Population / Client Focus

The ideal candidate will have a passion for working with a diverse patient population, including those experiencing orthopedic injuries, post-operative rehabilitation needs, and chronic conditions requiring physical therapy intervention. At Access Control Physical Therapy, your role is critical in closely monitoring patient progress, devising modifications to treatment plans as necessary, and ultimately enhancing quality of life through effective rehabilitation.

Qualifications

• Doctorate of Physical Therapy (DPT) from an accredited physical therapy program.

• Active state licensure as a Doctor of Physical Therapy.

• Minimum of two years of experience in outpatient orthopedic physical therapy.

• Current CPR certification.

Preferred Experience

• Experience with specific orthopedic conditions and rehabilitation protocols.

• Knowledge of evidence-based practices in physical therapy.

• Proficiency in electronic medical record (EMR) systems for efficient patient documentation and data management.
