Key Responsibilities\n

As a Licensed Clinical Social Worker, your day-to-day responsibilities will include:

• Conducting thorough psychosocial assessments to evaluate the mental and emotional health of clients.

• Providing individual, family, and group therapy using highly effective, evidence-based modalities such as Cognitive Behavioral Therapy (CBT), Solution-Focused Therapy, and Trauma-Informed Care approaches.

• Developing, implementing, and monitoring personalized treatment plans that address client needs, strengths, and goals.

• Collaborating closely with interdisciplinary teams, including psychiatrists and psychologists, to ensure integrated and holistic care for each client.

• Facilitating group therapy sessions, psychoeducation classes, and support groups tailored for patients and their families.

• Offering crisis intervention and risk assessment as needed, providing immediate support and safety planning to clients in distress.

• Maintaining accurate and timely documentation in compliance with ethical, legal, and organizational standards.

Patient Population / Client Focus\n

Your primary focus will be on individuals within the hospice care context, including patients facing terminal illnesses and their families. You will work to address mental health challenges such as grief, anxiety, and depression, equipping clients with coping strategies and emotional support as they navigate their circumstances. By providing ongoing support and therapeutic interventions, you will assist clients in achieving a sense of peace and acceptance regarding their experiences.

To be considered for this position, candidates must possess:
