Key Responsibilities

• Understand and adhere to state practice acts, ensuring compliance with all regulations governing the practice of physical therapy.

• Conduct thorough patient evaluations, utilizing specific assessments to create individualized treatment plans addressing functional limitations and disabilities.

• Implement evidence-based treatment protocols that align with evaluation findings, patient needs, and the current healthcare landscape.

• Maintain clear documentation of patient interactions, including initial evaluations, daily notes, progress summaries, and discharge evaluations, while ensuring accuracy and timeliness.

• Collaborate with other healthcare professionals to facilitate comprehensive care and make necessary referrals for patients requiring additional medical or rehabilitation consultation.

• Engage in effective discharge planning and follow-up care, ensuring that patients fully understand their treatment regimen and ongoing care needs.

• Participate in team meetings and contribute to the removal of barriers to patient access and engagement.

Patient Population / Client Focus

The Physical Therapist will primarily work with a diverse group of patients seeking rehabilitation for orthopedic issues, including post-surgical recovery, sports injuries, chronic pain, and other musculoskeletal conditions. Each patient presents unique challenges, requiring a tailored approach to treatment and rehabilitation.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution.

• Current state licensure as a Physical Therapist (PT).

• A minimum of two years of experience in an outpatient orthopedic setting.

• Current CPR certification.

Preferred Experience

• Experience with electronic health record (EHR) systems to streamline documentation and patient management.

• Previous involvement in multidisciplinary team settings to enhance patient care strategies.

Compensation and Benefits
