Work Environment

This role involves working directly in clients' homes, delivering physical therapy services tailored to individual recovery requirements. Key Responsibilities

• Directing and executing physical therapy treatment plans aimed at preventing disabilities, alleviating pain, and restoring muscle function for clients in accordance with their individual care plans.
• Collaborating with physical therapy assistants to ensure effective implementation of therapy interventions, while providing them with guidance and support according to the client's needs.
• Conducting thorough evaluations of the client's physical abilities through diagnostic tests focusing on muscle, nerve, joint function, and mobility.
• Assessing the client's home environment to identify adaptations that can enhance their independence and recommending appropriate modifications.
• Delivering hands-on therapy through manual therapeutic exercises, as well as educating clients and family members in functional and non-manual exercises.
• Administering further treatments such as traction and massage, coupled with physical agents, tailored to boost the client's recovery process.
• Fitting and adjusting prosthetic and orthotic devices, while collaborating with physicians to recommend necessary adjustments.
• Maintaining comprehensive clinical records, including initial evaluations, care plans, progress notes, and discharge summaries, to provide a transparent and detailed account of patient care.
• Participating in interdisciplinary team meetings and case conferences, contributing to comprehensive treatment plans and sharing expertise with fellow healthcare professionals.
• Being readily available for consultation with clients, families, and other agency personnel, ensuring a cohesive approach to therapy and care.
• Overseeing and mentoring Physical Therapy Assistants throughout therapy sessions, ensuring adherence to best practices and applicable laws.
• Completing functional assessments and revisions of care plans as clients progress, while ensuring effective utilization of community resources.
• Documenting all orders and clinical notes in a timely and organized manner, adhering to company policies for accuracy and compliance.

Patient Population / Client Focus

You will be working primarily with diverse patient populations, including elderly individuals and those recovering from surgery or injury who require tailored rehabilitation support in the comfort of their own homes. Your role will emphasize building relationships and trust with clients, facilitating their recovery in an encouraging and respectful way.

Qualifications

• Completion of a Doctorate in Physical Therapy (DPT) program is required.
• Possession of an active Physical Therapist license in New Jersey, with a clean record free from sanctions or disciplinary actions.
• Expertise in a range of manual therapy modalities and a proven ability to design effective exercise programs tailored to individual needs.
• Proficiency in managing electronic medical records, specifically using systems such as Web PT for effective documentation.
• Strong understanding of evidence-based physical therapy techniques that enhance patient outcomes.

Preferred Experience

• Previous experience in home health care settings is advantageous and will be considered favorably.
• Familiarity with interdisciplinary collaboration in treatment planning is preferred.
• Established track record of success in patient engagement and rehabilitation efforts.
