Key Responsibilities\n

• Conduct thorough assessments and ongoing evaluations to tailor care plans that meet the specific needs of each client.

• Facilitate regular communication with your team, including supervisors and care staff, to monitor client progress and adapt treatment plans as necessary.

• Develop and maintain a schedule for client appointments, ensuring that all in-home visits and therapy sessions are well-coordinated.

• Engage in collaborative home visits to carry out observations, interviews, and relevant data analysis.

• Ensure the effective implementation of program plans and provide oversight for the quality of services delivered.

• Participate proactively in Interdisciplinary Team Meetings, contributing insights and professional expertise.

• Document monthly and quarterly progress notes to monitor patient development and outcomes.

• Organize and conduct training sessions for team members to ensure the effective implementation of new procedures and protocols.

• Provide follow-up interviews and ongoing assessments to support and verify client needs post-intervention.

• Act as a liaison with the community and other healthcare professionals, enhancing client support and resources available within the mental health landscape.

Patient Population / Client Focus\n

This role involves working with a diverse client base, including adults and youth, each presenting unique mental health challenges. Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Arizona is mandatory.

• At least two years of relevant counseling experience in a clinical setting.

• Exceptional interpersonal and communication skills, with an ability to establish rapport with a diverse clientele.
