Key Responsibilities\n

• Conduct comprehensive assessments to tailor treatment plans to meet the unique needs of individual clients.

• Maintain effective communication with supervisors, direct care staff, family members, and guardians to discuss progress and address any concerns.

• Provide a structured schedule for client appointments, home visits, and observational assessments to ensure systematic tracking of client progress.

• Coordinate home visits that involve in-depth observation, chart reviews, staff interviews, and data analysis to enhance treatment effectiveness.

• Ensure effective execution of personalized treatment plans and monitor their implementation closely.
\n
• Engage actively in interdisciplinary team meetings as well as additional meetings necessary for treatment refinement and case management.

• Prepare and document monthly and quarterly progress reports to reflect client advancements.
\n
• Organize training initiatives to enhance staff competencies related to program implementation and client documentation practices.

• Conduct follow-up assessments via interviews and home visits to provide enduring support tailored to clients’ evolving needs.

• Maintain comprehensive client records and documentation to facilitate transparency and accessibility of information for all parties involved in care.

• Act as a liaison for community outreach and collaboration with other mental health professionals, supporting continuous community development goals.

• Contribute to the development of training materials aimed at improving programmatic delivery for clients.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a closely related field.

• An active and unrestricted license to practice as a Professional Clinical Counselor (LPCC) in California.
