Key Responsibilities\n

• Conduct thorough psychosocial assessments to identify the emotional, social, and financial needs of students and their families, developing individualized care plans based on these assessments.

• Provide one-on-one and group counseling, addressing issues related to grief, trauma, adjustments, and coping mechanisms.

• Coordinate with school personnel and community organizations to arrange for necessary post-discharge resources, ensuring seamless transitions for students.

• Engage in crisis intervention by offering immediate support and counseling to students and families during times of need.
\n
• Actively participate in interdisciplinary team meetings and hospital committees, contributing insights related to social work practices and advocating for patient-centered approaches.

• Maintain accurate, confidential, and timely documentation in compliance with legal standards and clinic policies.

Patient Population / Client Focus\n

The primary focus of this role is to support school-aged children from diverse backgrounds dealing with a range of challenges from academic pressure to family-related issues. Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Active state licensure as a Licensed Independent Clinical Social Worker (LICSW).

Compensation and Benefits\n
