What You’ll Do Provide Care Provide individualized substance use disorder case management that empowers patients throughout their recovery journey. Welcome and orient patients to treatment program services, policies, and expectations. Connect patients with internal and community resources through timely referrals and coordinated follow-up to ensure continuity of care. Provide crisis intervention and support during critical moments. Maintain knowledge of addiction treatment practices, regulations, and industry trends to deliver high-quality services. Manage Documentation Assess patients' social, educational, vocational, and recovery needs to support individualized care. Develop, update, and maintain patient treatment plans and comprehensive records. Complete accurate and timely documentation to support quality care, collaboration, and regulatory requirements. Team Building Assist with program implementation, process improvements, and caseload management. Represent the organization through community outreach and engagement activities. Foster a culture of inclusion, respect, collaboration, and belonging. Required Qualifications Must have a bachelor’s degree (or higher). Ability to meet applicable federal and state certification and licensure requirements. Preferred Qualifications Pursuing certification as a Certified Alcohol and Drug Counselor (CADC) preferred. Knowledge of addiction, substance use disorder treatment methods, and current trends in opioid maintenance treatment. Physical Requirements And Working Conditions Fast-paced clinical environment with period of increased activity. Exposure to patients with substance use disorders, mental health conditions, infectious diseases, and criminal justice involvement. Prolonged standing, bending, and stooping. Alternating between sitting and standing throughout the workday. Potential exposure to bloodborne pathogens. Use of appropriate personal protective equipment (PPE) when required.
