Responsibilities:
Assist our skilled nursing staff with providing direct patient care, including vital sign monitoring, bathing, dressing, and grooming
Support residents with activities of daily living (ADLs), such as feeding, toileting, and ambulation
Maintain a safe and clean environment by reporting any hazards or concerns to the nursing team
Develop and implement individualized care plans to meet the unique needs of each resident
Collaborate with other healthcare professionals to ensure comprehensive care coordination
