As a member of our Hospice Center team, you will operate in a remote telehealth environment, facilitating care for individuals in need across Kirkland, Washington, and surrounding regions. Key Responsibilities\n

• Deliver effective clinical services to eligible patients and families by employing evidence-based treatment models and best practice standards. You will be expected to achieve CANS certification within two months of your start date to ensure compliance and quality in care.

• Collaborate closely with other healthcare professionals, schools, and social service agencies to create comprehensive care plans that support continuity and maximize the quality of patient care. You will act as an integral part of the care team, ensuring that all aspects of the patient’s needs are addressed holistically.

• Assist clients in identifying resources, education, and strategies that encourage their independence and address their basic needs. This may include providing guidance on accessing community services or educational materials aimed at enhancing their overall quality of life.

• Monitor clients' adherence to court-mandated LRA (Least Restrictive Alternative) compliance and facilitate necessary extensions with the Crisis Response Team (CRT) and legal personnel as appropriate. This responsibility involves advocating for clients in legal contexts and working collaboratively with various stakeholders.

Patient Population / Client Focus\n

Your primary focus will be on patients and families facing terminal illnesses and complex psychosocial challenges. You will be tasked with understanding the unique dynamics of hospice care, enabling you to provide sensitive support tailored to different cultural and socioeconomic backgrounds. This role is crucial as it not only addresses mental health needs but also assists patients through their most vulnerable moments.

Qualifications\n

• A Bachelor’s degree in social work or related field is required, with a current LICSW license in Washington State.

• At least three years of direct clinical social work experience, demonstrating a thorough understanding of behavioral health assessment and intervention strategies.

• Strong skills in integrating behavioral health with primary care practices; training in this area will be provided if necessary.

• Familiarity with Medicaid, Medicare, and private insurance regulations, and the ability to navigate these systems effectively.

Preferred Experience\n

• Experience working with unsheltered or un-housed populations and a robust understanding of the challenges they face, along with knowledge of local resources.

Compensation and Benefits\n
