Key Responsibilities\n

• Collaborate closely with the healthcare team to develop effective treatment plans that address the psychosocial needs of patients and align with their overall care goals.

• Conduct thorough assessments of patients and their families, utilizing established social work interventions to inform discharge planning and care coordination.

• Maintain accurate clinical documentation, including progress notes and psychosocial assessments, in compliance with department policies and regulations.

• Provide crisis intervention services and counseling to patients experiencing emotional and behavioral problems, ensuring they receive the appropriate support.

• Engage in regular consultations with families and support systems to ensure a holistic approach to treatment and discharge planning.

Patient Population / Client Focus\n

You will be working primarily with patients facing end-of-life challenges in a hospice setting. This population requires a nuanced understanding of grief, loss, and the psychosocial nuances of terminal illness. You will be instrumental in providing emotional support and helping families navigate complex decisions regarding patient care, enhancing comfort, dignity, and quality of life during this critical phase.

Qualifications\n

• A Master's degree in Social Work from an accredited program is mandatory.

• Active licensure as a Licensed Independent Clinical Social Worker (LICSW) in the state of Washington is required.

Compensation and Benefits\n
