Key Responsibilities\n

• Utilize your education, training, and experience to independently assess and diagnose clients using psychosocial assessments, ensuring thorough understanding of individual circumstances.

• Collaborate with individuals and their families to develop tailored, patient-centered treatment plans that reflect their unique needs and goals.

• Implement evidence-based therapeutic interventions, including solution-focused therapy, motivational interviewing, and behavioral activation, to enhance clients' coping abilities and overall mental health.

• Work closely with an integrated team of healthcare providers, including psychologists and primary care physicians, to offer seamless support and holistic care that addresses the multifaceted needs of our patients.

• Continuously monitor patients’ progress through systematic tracking and regular check-ins, whether in-person or via telehealth, adjusting treatment plans as necessary to meet their evolving needs.

• Assist in the management of psychotropic medications as prescribed by medical professionals, focusing on adherence, side effects, and overall effectiveness of the treatment regimes.

Patient Population / Client Focus\n

This role will primarily serve patients facing critical end-of-life transitions, their families, and caregivers. You will be tasked with understanding and addressing the complex emotional and psychological challenges that arise in hospice care. Your ability to foster meaningful connections and provide empathetic support will be essential in navigating this sensitive environment.

Qualifications\n

• Master’s Degree in Social Work from an accredited institution.

• A current, valid, and unrestricted license to practice independently in mental health as a Licensed Independent Clinical Social Worker (LICSW) in Washington State.

• At least 1 year of experience as an independently licensed therapist, with a demonstrated ability to work with diverse populations and complex emotional situations.

Preferred Experience\n

Compensation and Benefits\n
