Key Responsibilities\n

• Collaborate with healthcare team members to formulate individualized treatment plans, including comprehensive discharge plans informed by psychosocial assessments and recognized social work methodologies.

• Conduct thorough assessments and implement appropriate psychosocial interventions to support patients during their care journey.

• Document all clinical interactions with precision, maintaining accurate and timely clinical notes and progress updates to adhere to departmental policies.

• Provide crisis intervention counseling, addressing emergent emotional and behavioral issues for clients in need.

• Engage with family members and support systems to coordinate client treatment, focusing on effective discharge planning and ongoing support mechanisms.

Patient Population / Client Focus\n

In this role, you will primarily serve a diverse patient population facing end-of-life challenges, grief, and chronic illnesses. Your expertise will be vital in helping clients and their families navigate impactful emotional and psychological experiences, as you support them through critical transitions and empower them with coping strategies.

Qualifications\n

• A minimum of three years of unsupervised, post-licensure experience in a clinical social work or counseling role.

• A Master’s degree in Social Work or Counseling from a regionally accredited institution.

• Current and unrestricted LICSW licensure in the state of Washington.

• Proficient in essential computer skills, including experience with electronic medical records (EMR), word processing applications, email systems, and spreadsheet software.

Compensation and Benefits\n
