Key Responsibilities\n

• Conduct thorough assessments of patients' psychosocial needs to create tailored treatment plans that reflect their unique circumstances and preferences.

• Implement evidence-based therapeutic interventions, such as individual counseling and family support sessions, to foster emotional healing and resilience.

• Maintain accurate, confidential records of client interactions and treatment plans in adherence to HIPAA regulations and organizational standards.

• Collaborate with physicians, nurses, and other healthcare professionals to ensure a holistic approach to patient care, actively participating in multidisciplinary team meetings.

• Engage in ongoing professional development by staying current with research and best practices in social work, particularly in the hospice and palliative care settings.

Patient Population / Client Focus\n

In this role, you will be supporting individuals facing terminal illnesses, as well as their families, by addressing emotional, spiritual, and psychological needs during a particularly vulnerable time. Your work will empower patients and families by providing them with coping strategies and resources, ensuring they are not alone on their journey. You will help clients navigate complex emotions, including grief, anxiety, and depression, while honoring their wishes and enhancing their quality of life.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.
