Key Responsibilities\n

• Engage in regular communication with physicians to monitor and report on patient progress, needs, concerns, and any potential issues that arise.

• Accurately maintain clinical documentation, including progress notes and treatment plans, in accordance with department policies and standards of practice.

• Participate in developing comprehensive treatment plans in collaboration with the healthcare team, including discharge plans informed by thorough psychosocial assessments and accepted social work interventions.

• Provide individual counseling and crisis intervention aimed at addressing and treating emotional and behavioral challenges faced by patients and their families.

• Consult with families and support systems to foster understanding and collaboration regarding client treatment approaches.

Patient Population / Client Focus\n

The social worker in this role will primarily serve patients in a hospice setting, which necessitates a deep understanding of the emotional complexities that arise when dealing with end-of-life issues. You will need to approach each case with sensitivity and empathy, providing dignified support to both patients and their families. This environment will require adept skills in dealing with grief, loss, and terminal illness challenges, ensuring that both patients and loved ones feel understood and supported.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution, specializing in Counseling and Mental Health.

• Active state licensure as a Licensed Independent Clinical Social Worker (LICSW) is required.

• A minimum of 3 years of experience working in an outpatient or hospice care setting.

• Proficient computer skills, including familiarity with electronic medical records and telehealth platforms, in addition to a solid grasp of MS Office applications.

• Exceptional organizational skills and the ability to work independently, managing one’s time effectively while remaining detail-oriented in documentation and communication.

Preferred Experience\n

While not mandatory, experience working with hospice or palliative care patients will be considered a strong asset.
