This position is entirely remote, providing the flexibility to work from home while still being an integral part of our Hospice Center's interdisciplinary team. You will engage digitally with colleagues and clients, utilizing our electronic health record (EHR) system and telehealth platforms to deliver care effectively. Key Responsibilities\n

• Conduct comprehensive psychosocial assessments to evaluate patients' mental health challenges, strengths, and support systems.

• Deliver tailored individual, family, and group therapy sessions that address the distinctive needs and cultural backgrounds of our clients.

• Utilize evidence-based therapeutic techniques, including cognitive-behavioral therapy, motivational interviewing, and family systems therapy to guide interventions.

• Develop and implement detailed treatment plans that include clear goals, tailored interventions, and measurable progress indicators.

• Collaborate closely with a multidisciplinary team—comprising psychiatrists, nurses, and support staff—to ensure holistic and coordinated care.

• Provide crisis intervention services, conduct thorough risk assessments, and develop safety plans as needed.

• Advocate for patients' rights, assist them in connecting with essential community resources, and offer comprehensive case management support.
