About the Role:

As a skilled and dedicated MDS Coordinator, you will be responsible for coordinating the Resident Assessment Instrument (RAI) process, ensuring accurate and complete Minimum Data Set (MDS) assessments, and implementing effective care plans. Your expertise will help us deliver high-quality care to our residents, while also promoting a positive and efficient work environment.

Responsibilities:

• Coordinate the RAI process, including scheduling, data collection, and completion of MDS assessments

• Ensure accurate and complete MDS assessments, while also addressing any additional documentation requirements

• Implement effective care plans, in collaboration with the interdisciplinary team, to meet the unique needs of each resident

• Maintain accurate and up-to-date records, including resident assessments and care plans

• Collaborate with staff to identify and address resident care needs, and implement changes as necessary

• Participate in quality improvement initiatives to improve resident outcomes and care processes

Requirements:

• Bachelor's degree in Nursing, Social Work, or a related field (n/a)

• 1+ years of experience in a long-term care or rehabilitation setting (n/a)
