Breaking Ground's Connect to Care program seeks a Case Manager to provide outreach and direct services to people experiencing homelessness, including those with mental illness and/or substance use disorders. The role focuses on helping clients move toward permanent housing through assessments, benefits assistance, and coordinated care. Reporting to the Social Services Supervisor, you will conduct psychosocial evaluations, connect clients to psychiatric, medical, and addiction treatment, and #J-18808-Ljbffr Breaking Ground
