The role includes patient education, support and coaching, ensuring services are in place for patient success. Responsibilities involve interacting, building, and expanding relationships with physicians, hospitals, skilled nursing facilities, senior agencies and other senior communities, delivering educational presentations and materials, facilitating support groups, conducting needs assessments and advanced care planning for senior living communities. The position is primarily based in our senior living communities and may require regional travel. Required Experience and Skills 1-3 years of health care experience preferred but not required. Social work or clinical liaison background preferred but not required. Strong technical skills in Microsoft Office Suite (Word, Excel, PowerPoint, Teams, Outlook). Strong presentation and public speaking skills. Good communicator, able to articulate product lines and services. Sales skills; solutions-oriented. Thrives in a fast-paced environment while remaining proactive, organized, and energetic. Proven self-starter with the ability to initiate, manage and complete multiple projects independently. Ability to improvise, multi-task and prioritize while maintaining composure in a dynamic environment. Demonstrates attention to detail and follow-through. Great time management skills. Exercise good judgment and sensitivity to confidential and non-routine matters. Primary Responsibilities Promote all CSIG service lines and drive referral business. Follow up with patients during hospital stay and after discharge. Provide patient coaching and support to develop personal health care plans. Conduct patient needs assessments. Facilitate support groups. Provide patient and professional educational services. Welcome and follow up calls for residents starting services. Work collaboratively with the community team to provide support and connectivity with other medical professionals. High level of teamwork required. Perform other tasks as needed. General Working Conditions The role requires effective communication, sitting, standing, walking, using hands for keyboard, telephone and other equipment. Occasional reaching, lifting or moving of up to 50 pounds may be required. The noise level is moderate. Some travel may be required.
