Community ACT Case Manager - Housing & Recovery Support
The Bridge, Inc.
The Bridge, Inc. is seeking an ACT Case Manager to provide intensive, community-based treatment and rehabilitation services to adults with serious mental illness in their homes and communities. Work as part of a multidisciplinary team, coordinating care with hospitals and community providers, and ensuring timely documentation and robust service planning.
