Key Responsibilities\n

• Conduct thorough biopsychosocial assessments to identify the mental health, substance abuse, and medical needs of patients within the hospice framework.

• Provide culturally competent and age-appropriate counseling services to individuals, couples, and families dealing with emotional distress and end-of-life issues.
\n
• Document mental health evaluations, progress notes, and treatment plans in accordance with regulatory standards and organizational policies.
\n
• Develop and execute crisis intervention strategies while assessing and addressing acute patient needs, promptly escalating to other providers when necessary.
\n
• Serve as a liaison between patients and community resources, enhancing access to medication assistance programs and supportive services outside the hospice setting.

• Collaborate with Quality Improvement teams to establish metrics for evaluating the effectiveness of our mental health interventions and ensure continuous improvement in patient care.

Patient Population / Client Focus\n

You will primarily serve elderly patients receiving hospice care, along with their families, providing invaluable emotional support as they navigate complex end-of-life issues. Your expertise will help these individuals find peace and comfort during difficult times, ensuring that their final experiences are as fulfilling and dignified as possible.

Qualifications\n

• Current and valid licensure as a Licensed Clinical Social Worker (LCSW) in the state of California is required.

• A Master’s degree in Social Work (MSW) is mandatory, coupled with a minimum of three years of post-licensure experience in clinical social work.

• Essential knowledge of counseling techniques and crisis intervention approaches, with a proven ability to make independent judgments in counseling settings.

Compensation and Benefits\n
